Self-Directed Servies

We are a Financial Management Service (FMS) provider that helps individuals self-direct their supports by managing payroll and other administrative responsibilities. Individuals want control over the direct care workers that come into their homes and decide upon the support plan to meet their needs. UCP of West Central Wisconsin makes a difference by supporting individuals and families to have more control and choice over who, what, when and how those services are provided.

Fiscal Agent Service

UCP of West Central Wisconsin serves as a fiscal employer agent for children and adult programs in the State of Wisconsin. This is often referred to as Self-Directed Services. Self-direction simply means that individuals can hire their own employees and choose how the work is done in their home on a regular basis. The individual is responsible for training, scheduling and assuring that authorized services are being done according to their care plan. UCP of West Central Wisconsin manages enrollment, onboarding paperwork, communication with Service Coordinator/Case Manager for set-up, tracking utilization of the individual’s budget, completing bi-weekly payroll and tax payments. Using our fiscal agent service, individuals can trust that their workers will get pay on time and other administrative burdens are properly handled.

Fiscal Conduit

UCP of West Central Wisconsin provides payment to vendors and agency providers who provide goods or services to the individual in accordance with their authorized budget and care plan.

Which programs does UCP of West Central Wisconsin work with in the State of Wisconsin?

We currently work with Children Services or County CLTS programs and Family Care Managed Care Organizations.

The Children’s Long-Term Support (CLTS) Program is a Home and Community-Based Service (HCBS) waiver that funds community supports and services for children who have substantial limitations in their daily activities and need support to remain in their home or community. Eligible children include those with developmental disabilities, severe emotional disturbances, and physical disabilities. When a child on CLTS that is using us for FMS becomes 18 years of age then many of them transition to adult services in the State of Wisconsin. UCP of West Central Wisconsin works closely with this transition process to make sure self-directed care and supports isn’t disrupted from child to adult service systems.

Family Care is a Medicaid long-term care program for frail elders and adults with physical, developmental, or intellectual disabilities. Individuals in the program receive long-term care services to help them live in their own home.

Family Care has a choice of Managed Care Organizations (MCO) for individuals to pick from and each MCO has Service Coordinators/Case Managers to help develop a care plan for needed services or supports. This includes routine tasks such as bathing, getting dressed, making meals, going to work, and paying bills. Please click here to learn more about Family Care.
